District Nursing and Home Based Care

The District Nursing Service is a home based nursing service providing care in the community. South Gippsland Hospital offers a daily service, between 8am and 4.30pm Monday to Friday and 8am to 12 noon on Saturday and Sunday.

Patients can be referred by nursing or medical staff or self refer.

Fees

Fees are charged on a sliding scale, with a small fee charged for pensioners and health care card holders. A small fee for wound care dressings may be charged by nurses who provide care for clients in the community.

  • DVA- Department of Veteran Affairs
  • TAC –Traffic Accident Commission
  • Package funded
  • HACC – Home and Community Care
  • PAC – Post Acute Care
  • HITH – Hospital in the Home
  • Palliative Care

Weekly Clinic

The DNS also provide a weekly Wednesday clinic at the Community Health Centre for patients who require general nursing care such wound care, vital signs, foot care, catheter changes and other health related requirements.
