District Nursing and Home Based Care

The District Nursing Service is a home based nursing service providing care in the community. South Gippsland Hospital offers a daily service, between 8am and 4.30pm Monday to Friday and 8 am to 12 noon on Saturday and Sunday.

Patients can be referred by nursing or medical staff or self refer.

Fees

Fees are charged on a sliding scale, with a small fee charged for pensioners and health care card holders. A small fee for wound care dressings may be charged by nurses who provide care for clients in the community.

  • TAC –Traffic Accident Commission
  • Package funded
  • HACC – Home and Community Care
  • PAC – Post Acute Care
  • HITH – Hospital in the Home
  • Palliative Care

Weekly Clinic

The DNS also provide a weekly Wednesday clinic at the Community Health Centre for patients who require general nursing care such wound care, vital signs, foot care, catheter changes and other health related requirements.
